You can print it out from your own computer, complete it, and submit it to us. Do you have a piece of historical property you need or wish to work on in some way? Make sure you review the Secretary of the Interior's Standards; then you can use the Application for Certificate of Appropriateness for Rehabilitation of a Historic Property. The application is in PDF format, you so you will need software such as the Adobe Reader to use the file. You can download it free from Adobe.
